2. Program Acknowledgment

This is a peer-supported recovery residence, not a licensed treatment facility. By signing this agreement, Resident voluntarily agrees to reside at Recovery & Revival Connection and participate in the recovery-oriented structure of the home. Resident understands that continued residence is contingent upon compliance with all terms of this agreement.

3. Financial Terms

Rent is due on the schedule specified above. A late fee applies if rent is not received within 3 days of the due date. The program deposit is refundable upon move-out in good standing, less any deductions for damages, unpaid rent, or outstanding fees. Residents discharged under Section 6 (Immediate Discharge) forfeit their deposit.

4. House Rules & Expectations

a. SOBRIETY — Zero tolerance for alcohol or illegal drug use, on or off premises. This includes possession, distribution, or being under the influence.

b. DRUG TESTING — Resident consents to random drug and alcohol testing (urinalysis, breathalyzer, or other methods) at any time. Refusal to test is treated as a positive result.

c. MEETINGS — Resident is required to attend a minimum of 3 recovery meetings per week (12-step, faith-based, or approved equivalent). Proof of attendance may be required.

d. CURFEW — Weekday curfew: 10:00 PM. Weekend curfew: 11:00 PM. Exceptions must be approved in advance by house management.

e. CHORES — Resident will complete assigned household chores on schedule. Assignments rotate as determined by house management.

f. GUESTS — Visitors are permitted only during approved hours. No overnight guests. All visitors must be approved by house management.

g. CONDUCT — No violence, threats, intimidation, theft, weapons, or sexual misconduct. Mutual respect for all residents and staff is required at all times.

h. QUIET HOURS — Quiet hours are from 10:00 PM to 7:00 AM daily.

i. PERSONAL SPACE — Resident is responsible for maintaining cleanliness and hygiene of their assigned room and personal areas.

j. MEDICATION — All prescription medications must be disclosed to house management and stored as directed.

5. Progressive Discipline

For violations not listed in Section 6, the following progressive steps apply:

Discharge from the residence

House management reserves the right to skip steps for serious or repeated violations.

6. Grounds for Immediate Discharge

The following will result in immediate removal from the residence with no refund of deposit:

Use, possession, or distribution of alcohol, illegal drugs, or non-prescribed controlled substances
Violence, assault, or threats against any person
Possession of weapons of any kind
Sexual misconduct or harassment
Criminal activity on or off premises
Refusal to submit to drug or alcohol testing
Tampering with or falsifying drug test results

7. Termination & Move-Out

Voluntary Move-Out: Resident agrees to provide at least 7 days written notice before vacating.

Involuntary Discharge: House management may terminate this agreement at any time for violations of this agreement.

Abandoned Property: Personal property not retrieved within 7 days of move-out will be considered abandoned and disposed of.

Deposit Return: The program deposit will be returned within 14 days of move-out, less any deductions for damages, unpaid rent, or outstanding fees.

8. Liability & Hold Harmless

Resident agrees to hold harmless and indemnify Recovery & Revival Connection, its owners, operators, staff, and agents from any and all claims, damages, losses, or injuries arising from Resident's participation in the program, actions of other residents, or use of the premises.

The facility is not responsible for loss, theft, or damage to Resident's personal property. Resident acknowledges that recovery housing involves inherent risks and voluntarily assumes these risks.

9. Confidentiality

Recovery & Revival Connection will maintain the confidentiality of Resident's personal information and will not disclose participation in the program without written consent, except as required by law or in the event of a medical emergency.

10. Acknowledgment & Signatures
